At initial assessment please bring:

  • Physician referral
  • Appropriate attire (shorts, t-shirt, tank top, and clean footwear)
  • List of prescribed medications
  • Claim number for SGI or WCB (if applicable)
  • Health Card

Parking available in designated spots at side of building.
